should the kote and do be worn tight or loose or just nice?

The only advice I can give you about the do is to wear it as high as possible.
Sometimes people hit too high and the blow lands higher than the do, that can be painful.

Just nicely will do for the kote, I think.

I like it loose. Allows me a greater range of motion. Although i've seen some people wear it really tight. It's just personal preference.

Wearing the kote a little loose will absorb hits a little easier...
tight kote = ouch ... when you get hit with a good strike..
